BabyTime Enrollment Forms
Welcome to BabyTime Family Childcare! We are excited to be a part of your child’s early learning experiences!
To start the enrollment process, please submit an application for each child. Once your application is reviewed, we will send you additional information including all the required enrollment forms below. Please take your time to review, complete, and send all forms back to us via email or schedule a time to drop them off.
There is a non-refundable enrollment fee of $100 per child. We need to review and confirm all required forms for each child before they can attend BabyTime Family Childcare.
We want you and your child to be pleased and comfortable with your decision to join BabyTime. Therefore, we offer a 2-week trial period, in which parents and providers reserve the right to terminate the contract agreement without advance notice.
